If you want to request your own personal information, you should use the form below. Please complete the form and return it to the Data Protection Officer. You will be asked to provide proof of identity, this can be a copy of your passport, driving licence, birth certificate or some other official document bearing your signature. In some cases we will also require a recent photograph of you for elimination purposes.
